Employee Benefit Plans
Employers who are self insured experience immediate benefit when employees choose less costly medical procedures to meet their needs.  Those with indemnity plans reduce their claims experience.  Some companies may wish to negotiate with insurers for lower premium rates for those using My Life Preferences™. Employers may incentivize using MLP as a way of reducing unwanted medical procedures.

The cost of one procedure to treat a terminal illness can range from $50,000 to $150,000.  That means employers can recover the cost of a license when one employee or covered dependent chooses to forego a futile end of life procedure. Examples include general surgery, organ transplants , cancer surgeries,  cost of additional hospital days, reconstructive   surgery,  rehabilitation,  medications, follow-up chemotherapy and/or  radiation  therapy, medical equipment, and home health care. When employees use My Life Preferences™, they control their decisions.    Some will wish to use all available means in hope of surviving.  Others may forego treatment and choose palliative care to make the most of their remaining time.   Both choices are right.

*To purchase a license with no expiration date, add 50% to the prices listed above. Where applicable, a Wisconsin Sales Tax of 5.5% will be added to the price.  All licenses include free upgrades for the duration of the license.

Insurance Companies

Health Care Facilities
have two options: 1) provide MLP for use only by their staff or 2)  purchase an expanded license authorizing them to reproduce copies for use by their physicians. Send queries to info@mylifepreferences.com.

Places of Worship
Religious congregations wishing to offer this service to their members can purchase a license for a onetime fee of $1500.  Send queries to info@mylifepreferences.com.

Attorneys
Attorneys can use MLP to build upon formats for living wills and power of attorney required within their State of jurisdiction.  Doing so offers clients a much more comprehensive package.  Attorneys interested in using My Life Preferences™ in this way can purchase an individual license or group license. Family Package
Cost US $100.  (5.5% Wisconsin sales tax will be added for Wisconsin residents.) 
A family package grants spouses, adult children, spousal parents and grandparents, the exclusive right to reproduce and use this instrument. Family package purchasers will receive their product as a PDF attachment showing their family name and nontransferable license number.  Family packages do not include free upgrades.
